North Crest’s Policies
Release of Liability
In mutual consideration for membership in North Crest Kids Acticity Center, on behalf of myself and my immediate family, I hereby fully and forever release and discharge North Crest Kids Activity Center, its officers, shareholder, employees, directors and agents from all claims, demands, damage action or injuries, whether any injury or damage be known now or in the future, whether anticipated or unanticipated, resulting from or arising out of my child's use or intended use of the facilities, equipment and services. I agree on behalf of myself, my heirs, assign and minor(s), to hold North Crest Kids Activity Center harmless from any claim for damages arising out of any accident or injury that may be caused to myself or my child. I understand I am responsible for my child up until class start time and immediately after class ends.
Assumption of Risk
I recognize that participation in gymnastics and activities involving movement exercise carries with it the risk of catastrophic injury.
Medical Emergency
The undersigned authorizes North Crest to act for me according to their best judgment in any emergency requiring medical attention including the service of a rescue squad and/or emergency room. This authorization shall remain effective until revoked in writing.
Photo Release
I agree to allow North Crest to publicize in any manner any photo or video of me or my child at any time.
Annual Membership Fee
I understand that I will be charged an annual registration fee. This fee allows me to participate in North Crest classes and events from September through August. As a member I will receive discounts on many of North Crest's peripheral programs such as Birthday Parties, Open Gyms, and Parent Night Out. I understand that this fee is $30.00 and will be pro-rated according to the first month I attend by $2.00 per month. Example: Sept $30, Oct. $28, Nov. $26, etc.
Credit/ Debit Card and Tuition Policy
It is required that you supply a credit card when registering. (This card will be kept on file, however, due to federal laws, and for your safety, no one will be able to view your information. The information becomes stored in the software and only the last 4 digits are viewable.) The card will be charged upon receipt of your registration for your annual family membership fee and your first month’s tuition. Tuition is due the 15th of the previous month of service. Be aware that if you register on or after the 15th, you will be charged your registration fee, your pro-rated first month’s tuition and your second month’s tuition, as it would be due at this time. You may pay your tuition via cash or check before the 15th of the previous month to avoid having your credit card charged. If your payment has not been received, your credit/debit card will be charged for any remaining balance on the 16th or closest business day. If your card has expired or is declined and payment is not received by the 25th you will receive a $10 late fee. If payment has not been received by the end of the month prior your child may be dropped from their class/es.
Absence Policy
If you are unable to attend one of your classes, please call the office prior to your absence. A coupon will be issued for one of the following: 1 free open gym or $10.00 off Parent Night Out (one child only) during the regular season; A coupon for 1 free open gym or 50% off 1 day of Camp North Crest during our summer season. If your class is canceled due to weather conditions, a coupon will be issued as previously stated. Tuition will not be adjusted for absences or missed classes.
Discontinue/ Drop Policy
I understand the program I have registered for runs in monthly billing segments continually through August and I must give a written notice prior to the 15th of the previous month in order to drop the program and thereby eliminate tuition responsibility. I also understand that my tuition will not be adjusted for discontinuing classes mid-month.
